CANCELLATION & REFUND POLICY
Your appointments are very important to the employees of Air, Land & Sea Physicals & Consultation, LLC (ALS). Your time slot is reserved exclusively for you. We understand you’re your flight may change and sometimes schedules adjustments are necessary; therefore, we respectfully request at least 24 hours of notice for cancellations. Examinees must cancel testing appointments ONLINE OR VIA EMAIL.
Due to testing scheduling, staffing, resources and lab agreements refunds will not be offered to those that cancel with less than 24 hours of notice. Cancellations are accepted at any time up to your appointment day at which point if you cannot attend your requested time slot you can either forfeit your appointment or reschedule. As a reminder, we send reminder emails and text message (US Cell Phones). There is no fee for changing your registration online, and you may change your registration more than once. However, payment may be required when processing certain changes (e.g., changing from a less expensive test to a more expensive test).
STRICT AND ENFORCED 24 HOUR CANCELLATION POLICY
Please understand that when you forget or cancel your appointment without giving enough notice, an opportunity is lost to fill that appointment time and patients on our waiting list miss the opportunity to receive services. Our appointments are confirmed via multiple emails and reminders in advance. Since the services are reserved for you personally, a cancellation fee will apply. If you fail to take a test for which you are registered on a scheduled test date, no fees or charges will be refunded.
HOW TO CANCEL
Please cancel the appointment via the link sent with the appointment confirmation in the e-mail. Refunds will be issued automatically and can take 10+ days to appear on your statement, if it takes longer please contact your bank for assistance.
Please note: If you have registered for a test that is scheduled to begin within 24 hours, that appointment cannot be cancelled, and no refund or transfer of fees will be provided.
Cancellation Fees:
· “NO SHOWS” will be charged 100% of the reserved service amount.
· Same day cancellations will be charged 100% of the reserved service amount.
Rescheduling:
We offer unlimited free rescheduling of your appointment to any given time slot.
Insurance Reimbursements:
ALS does not offer refunds for costs of services that have been denied by your insurance carrier. All sales are final. Any reimbursement that you attempt to receive for the cost of your test should be taken up with your insurance carrier. ALS does not offer refunds for any reason including matters related to health insurance reimbursements.
As a reminder, only United States based health insurance carriers qualify for the insurance rate listed on our website.
Refund Policy:
Patient agrees that all transactions are non-refundable, unless adhered to cancellation policy. ALS agrees to render services as purchased by patient. Patient understands that delays or situations that are out of our control are not grounds for a refund. Satisfaction is not determined by results or timing of reports. it is the customers responsibility to follow up with the lab to find out if the results have been sent out correctly. All inquiries for not receiving results by time they are due shall be directed to covidresults@air-land-sea.com with the subject line “NO RESULTS”. ALS is not responsible for results ending up in Spam or being delayed.
All services are non-refundable.
